Changing the Narrative: Building a Healthier Community Together

People typically associate clinics and hospitals with illness, injuries, and emergencies. Tioga Medical Center is working to change that narrative with the addition of a new role focused on prevention and wellness —Population Health Coordinator, Lanae Scott.
In her role, Lanae identifies patients who have gaps in their care, meaning they may be missing important screenings or wellness visits. She then reaches out personally to help them schedule those services, making preventive care easier and more accessible.
“Life is busy, and it’s sometimes hard to keep track of when you’re due for this screening
and that screening,” says Lanae. “We want to be that gentle reminder to take care of
yourself, to get in for those important screenings, and to make your health a priority.”
This proactive approach is part of Tioga Medical Center’s broader effort to enhance access to care for the region. Just this year, the organization has welcomed three new providers — David Petrosian, DO, Medical Director; Jennifer Reynolds, FNP; and Kari Ely, FNP —bringing the total number of medical providers on staff to seven, a milestone first for the facility.
“We are committed to expanding access to high-quality care, whether that care is
preventive in nature or treatment of injuries or illnesses,” says Jamie Eraas, Tioga Medical Center President and CEO. “The addition of these new providers means more appointment availability and enhanced capacity to see patients for wellness purposes.”
Tioga Medical Center’s ongoing growth and emphasis on population health reflect a simple but powerful belief — that healthcare is not just about treating illness, but about nurturing wellness, connection, and peace of mind for every patient served. From preventive screenings and immunizations to chronic disease management and healthy lifestyle support, the focus is clear: helping community members stay well, not just get well.
